2009 Fall Sports Physical Appointments
Must Sign-up in person or call the School Nurse to set up an appointment with the School Physician by Wednesday, May 27

Physicals are:
Wednesday, June 3 and Tuesday, June 9
After school in the High School nurse's office
Deadline for all HS Fall Physicals is Friday, July 31

Please keep in mind that submitting forms late
