Section 2 - Required District Forms:
Certificate of Residency
Required of all parents/guardians. One per child.
Standard Residency/Registration-Form 100
Required of all parents/guardians. One per child for pages 1 & 2. Pages 3&4 per family.
Section 3 - Required Forms Dependent Upon Situation:
Residency Certification for Divorced Parents
To be used when the parents are divorced and have 50/50 custody.
To be used by the resident who is renting or leasing a home.
To be used when the resident with whom the applicant is living with, is not the owner or landlord of the property.
To be used if student is living with a parent/guardian temporarily residing within the district.
Homeless, DYFS, or court order placement form
Non-Rent Paying Residency Affidavit-Form 109-110
To be used if parent/guardian does not pay rent or own the home for the premises in which they reside.
Affidavit of Student Enrollment-Form 200-A
To be used if the student is living with a person domiciled in the district, other than their parent/guardian. Must also complete Affidavit of Parent/Guardian & Affidavit of Resident Form 200-2 and 200-2b
Affidavit of Parent-Guardian-Form 200-1
Establishes the existence of a family or economic hardship which renders the parent/guardian is incapable of providing support or care of the pupil
Affidavit of Resident-Form 200-2
Establishes the existence of a family or economic hardship which renders the resident is incapable of providing support or care of the pupil
Establishes the existence of a family or economic hardship which renders the parent incapable of providing support or care of the pupil.
Section 4 - Change of Address Form:
Needed if you are moving within Madison as well as all the necessary residency forms above that relate to your situation (rent or own a home).
