Carrie Guarino, our full time school nurse, is available for our students first aid needs, illnesses, and to assist with both acute and chronic health conditions. She also conducts annual health screenings and provides referrals as needed.

ATTENDANCE: If your child will be absent or late, please call the roll call line at 973-593-3182 (option 1) each morning before 8:30 a.m. Leave your child’s name and the reason for the absence. Please make every effort to schedule medical appointments early in the morning or later in the afternoon, as arrival to school after 11:00 AM or dismissal prior to 1:00 PM will negatively impact their attendance. 

SCHOOL ENTRY REQUIREMENTS:

Immunizations: The following immunizations are required for school entry and proper documentation (including month, day and year of administration) must be submitted PRIOR to the first day of school. Per NJDOH regulations, students who are not in compliance must be excluded from school until the required immunizations are completed: 

  • DTaP: 4 doses, with 1 dose after the 4th birthday OR any 5 doses

  • Polio: A total of 3 doses with 1 dose after the 4th birthday OR any 4 doses

  • MMR: 2 doses

  • Hepatitis B: 3 doses

  • Varicella: 1 dose (or documented disease)

  • Meningococcal and Tdap: Administered once the student has turned 11 and prior to entry into 6th grade

Universal Health Record: A physical exam is required for all new students and must be completed by your child’s healthcare provider for school entry. A  30 day grace period may be applied. 

EMERGENCY MEDICATIONS: Students with asthma, allergies, diabetes, or seizures who require medication in the nurse’s office must submit an Emergency Action Plan completed, signed and stamped by their healthcare provider.All emergency medications must be in the original pharmacy-labeled container and be brought to school in a zip-top bag with a small photo of your child. ADDITIONAL MEDICATIONS: For non-emergency medications, including ADHD medications and over-the-counter products such as Advil, a Medication Form is required. This form must: be completed by your child’s healthcare provider, signed by the parent/guardian, and clearly state that the school nurse is authorized to dispense the medication. All medications must be submitted in the original pharmacy-labeled packaging and brought to school by a parent or guardian.

ILLNESS: Please keep your child home from school if they aren’t feeling well, or have had a fever, vomiting or diarrhea within the past 24 hours, and report all diagnosis to the school nurse. A note from a healthcare provider is generally required to excuse any absence beyond 3 days. Please see attached K-12 NJDOH school exclusion list:

HEAD LICE (PEDICULOSIS) INFORMATION: The American Academy of Pediatrics and CDC strongly discourage head lice screenings in the school setting as they can be stigmatizing and have not been proven to reduce the incidence of head lice over time.  Since head lice circulates throughout the year, it is important to check children regularly and report any positive cases to the school nurse.
