MEDICATIONS

All non-emergency medications, including ADHD medications and over-the-counter products such as Advil, nutritional supplements, eye drops, etc. require written authorization from a physician. This Medication Form must be completed by your child’s healthcare provider, signed by the parent/guardian, and clearly state that the school nurse is authorized to dispense the medication. All medications must be submitted in the original pharmacy-labeled packaging and brought to school by a parent or guardian.

EMERGENCY MEDICATIONS

Students with asthma, allergies, diabetes, or seizures who require medication in the nurse’s office must submit an Emergency Action Plan completed, signed and stamped by their healthcare provider. All emergency medications must be in their original pharmacy-labeled container and be brought to school in a ziplock bag with a small photo of your child.
