Diane Fastiggi, our full-time school nurse, is available for our students first aid needs, illnesses, and to assist with chronic health conditions. Please stop in, call 973-593-3117 ext. 3, or email fastiggid@madisonnjps.org with any questions or concerns. Email is the best way to reach the nurse during the summer months.
Please keep your child home from school if they aren’t feeling well, or have had a fever, vomiting, or diarrhea within the past 24 hours.
Attendance/Tardiness: If your child is absent or late coming to school, please call the Roll Call number at 973-593-3117 ext. 1 each morning with your child’s name and the reason for absence.
New students: a current universal health record completed within the year and immunization record are required to attend school.

Required Immunizations:
Emergency Medication: Students with asthma, allergies, diabetes or seizures requiring medication to be kept in the nurse’s office must provide an Emergency Action Plan from your healthcare provider. All medication should be in the original pharmacy labeled container, and brought to school in a zip top bag with a small photo of your child. Students that have EpiPens and inhalers are permitted to carry their own medication, but require physician and parent authorization using the Authorization to Self Carry and Indemnification Forms. Any medication misplaced by a student and found on school premises will result in school disciplinary action.
Other medication: If a student is required to take prescription or over-the-counter medication during the school day, it must be brought to the nurse in the original pharmacy container (prescription) or unopened bottle (over-the counter) and accompanied by the Request to Administer Medication Form. Medication may be given only by the school nurse and should not be carried by students at any time.
Physical Education Excuses: All students must take Physical Education as mandated by the NJ Department of Education unless he or she has a signed doctor’s note. A note from the parent will excuse the student for 2 days. Notes will only be accepted from a medical physician, physician’s assistant or nurse practitioner for extended absences. We cannot accept gym excusal notes from a physical therapist, or chiropractor.
MHS Sports Program: Registration for athletics is on Arbiter Sports. The History and Physical Examination forms should be kept on file by the healthcare provider and only the Medical Eligibility form should be uploaded in Arbiter. Athletic forms are valid for 1 year from the date of the physical examination. All sports physicals should be completed by the students primary medical doctor whenever possible. Students must be approved by the District Physician before he or she can try out for a sport. Please note that students who submit paperwork for physicals after deadlines have passed are not guaranteed clearance for participation for their first day of practice.
